The Opportunity

We are currently seeking a dedicated Nursing Home Hospital Liaison

What You Will Do

• Build and maintain strong relationships with hospital case managers, social workers, discharge planners, and physicians
• Work closely with our centralized review department to execute a bedside visit with potential admissions candidates (PAC)
• Present diagnosis specific marketing collaterals to potential nursing home admission candidates and influencers
• Coordinate facility tours and interaction between PAC/influencer and facility specific staff
• Collaborate with facility admission teams to ensure efficient and timely admission processes
• Identify and overcome barriers to admission, including clinical, insurance, and logistical concerns
• Provide a full time presence in designated hospitals
• Inform hospital staff, PAC and influencers on Harborview’s clinical capabilities and differentiation between levels of care
• Duties as assigned

What You Must Have

• Active RN/LPN license (or related) in good standing in the state of practice.
• Experience in a skilled nursing or long-term care setting is preferred but not required—we’ll invest in the right heart and mindset.
